To fill out this form, you will need to following:

1. Credit Card (If you cannot join with a credit card, and you must use a check, then you will pay the current fee plus a $30 processing fee).  To join with a check, you will need to email kscamail@yahoo.com 

The form will ask you for the following information:

2. School class (1A, 2A, or 3A - based on the current track and field classification.  See https://www.kyhighschoolsoftball.com/members if you don't know yours)

3.  Region (1-16 based off the softball regions - not track and field.  See https://www.kyhighschoolsoftball.com/members if you don't know your region)

4. School Name (do not list "high school")

5. Head Coach's Name

6. Heach Coach's Email address for listing on the website (if you want it listed)

7. Head Coach's Cell Phone for the listing on the website (if you want it listed)

8. Coaching Staff Names (up to 5) if you are paying for Head Coach plus staff.

IMPORTANT NOTE: THE EMAIL YOU USED FOR YOUR SPORTSENGINE ACCOUNT WILL BE THE EMAIL THAT WE USE TO COMMUNICATE IMPORTANT INFORMATION THROUGHOUT THE SEASON.  YOU HAVE THE ABILITY TO CHANGE YOUR SPORTSENGINE EMAIL WHEN AND IF NEEDED.
